การวิเคราะห์การจัดสรรงบประมาณยาจากโรงพยาบาลวัฒนานครสู่โรงพยาบาลส่งเสริมสุขภาพตำบล

Abstract

Sub-district health promoting hospitals receive allocated budgets for drugs and medical supplies from partner hospitals of the primary care unit in various forms depending on the policy of each location. The budgets for drugs and medical supplies is varied according to the performance of sub-district health promoting hospitals, but the previous allocation of budgets for drugs and medical supplies from Watthananakhon Hospital did not consider the performance. However, if a variable is found related to the drug expenditures it will be useful to use as a guideline for drug budget allocation. Objective: To find the variables affecting drug expenditures. Methods: It is an analytical study. Collecting information of all sub-district health promoting hospitals in Watthananakhon District, Sa Kaeo Province during the financial year 2015-2018, analyzed the relationship between independent variables and drug expenditures by correlation analysis and analyzed the relationship of the independent variables on the dependent variable using the Generalized Linear Model (GLMs). Results: The study found age group, sex, service type, sub-district health promoting hospitals size, number of service personnel, distance and public transportation were significantly associated with drug expenditures (p-value 0.001). Conclusion: The independent variables that was found to correlate with drug expenditures can be applied to forecast drug budget allocation.

โรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ได้รับการจัดสรรงบประมาณด้านยาและเวชภัณฑ์จากโรงพยาบาลคู่สัญญาของหน่วยบริการปฐมภูมิหลากหลายรูปแบบขึ้นกับนโยบายของแต่ละแห่ง โดยงบประมาณด้านยาเป็นงบดำเนินการที่แปรผันตามผลงานของ รพ.สต. ซึ่งจะเกี่ยวข้องกับค่าใช้จ่ายด้านยาที่ใช้ในการให้บริการประชากร แต่การจัดสรรงบประมาณด้านยาจากโรงพยาบาลวัฒนานครสู่ รพ.สต.ที่ผ่านมาไม่ได้พิจารณาจากผลการดำเนินงาน อย่างไรก็ตาม หากพบตัวแปรที่เกี่ยวข้องกับค่าใช้จ่ายด้านยาของ รพ.สต.จะเป็นประโยชน์ในการใช้เป็นแนวทางการจัดสรรงบประมาณยาได้ วัตถุประสงค์ เพื่อค้นหาตัวแปรที่ใช้ทำนายค่าใช้จ่ายด้านยา วิธีการศึกษา เป็นการศึกษาเชิงสหสัมพันธ์ เก็บข้อมูลประชาชนทุกรายที่มารับบริการที่ รพ.สต. ทุกแห่ง ใน อ.วัฒนานคร จ.สระแก้ว ในช่วงปีงบประมาณ 2558-2561 วิเคราะห์ความสัมพันธ์ตัวแปรต้นกับค่าใช้จ่ายด้านยาด้วยวิธีวิเคราะห์สหสัมพันธ์ และวิเคราะห์ความสัมพันธ์ของตัวแปรต้นที่มีผลต่อตัวแปรตามโดยใช้โมเดลเชิงเส้น (Generalized Linear Model) ผลการศึกษา พบตัวแปรต้นที่มีความสัมพันธ์กับค่าใช้จ่ายด้านยาอย่างมีนัยสำคัญทางสถิติ 0.001 ดังนี้ กลุ่มอายุ เพศ ประเภทบริการ ขนาดของ รพ.สต. จำนวนเจ้าหน้าที่ผู้ให้บริการ ระยะทางความห่างไกล และการขนส่งสาธารณะ สรุป ตัวแปรต้นที่พบความสัมพันธ์กับค่าใช้จ่ายด้านยา สามารถนำมาปรับใช้ในการพยากรณ์การจัดสรรงบประมาณยาได้
