ปัจจัยที่มีอิทธิพลต่อการตัดสินใจใส่ฟันเทียมของผู้สูงอายุ อำเภอลานกระบือ จังหวัดกำแพงเพชร

Abstract

This descriptive study aimed to determine the decision to wear dentures and factors influencing the decision-making in dental prosthetic treatment of the elderly in Lan Krabue District, Kamphaeng Phet Province. The sample group consisted of 283 people using a multi-stage random sampling. Data were collected using questionnaires. The statistics used to analyze the data were frequency, percentage, mean, standard deviation, and binary logistic regression analysis.

The results showed that most of the elderly decided to wear dentures (53.7%). Factors influencing the decision-making in dental prosthetic treatment were perceived need for dentures (ORAdj=21.69; 95%CI=9.46-49.70; p<0.001), expectations regarding dental prosthetic treatment services utilization (ORAdj=7.24; 95%CI=2.44-21.44; p<0.001), influence from dental personnel (ORAdj=4.63; 95%CI=2.00-10.69; p<0.001), motivation for wearing dentures (ORAdj=3.45; 95%CI=1.39-8.57; p=0.008) and residential area (ORAdj=2.83; 95%CI=1.04-7.66; p=0.041). These factors influenced in predicting the decision-making in dental prosthetic treatment of the elderly by 75.10%.

This study suggests that the hospital should provide dental prosthetic treatment services in sub-district health promotion hospitals to increase access to dental prosthetic treatment services for the elderly. Moreover, dental personnel should provide advice, knowledge, information about dental prosthetic treatment, and good services to the elderly to increase the chances of making a decision to wear dentures for the elderly.

การศึกษาเชิงพรรณนานี้มีวัตถุประสงค์เพื่อศึกษาการตัดสินใจใส่ฟันเทียม และปัจจัยที่มีอิทธิพลต่อการตัดสินใจใส่ฟันเทียมของผู้สูงอายุในอำเภอลานกระบือ จังหวัดกำแพงเพชร กลุ่มตัวอย่าง จำนวน 283 คน ใช้วิธีสุ่มกลุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ คือ แบบสอบถาม สถิติที่ใช้ในการวิเคราะห์ข้อมูล ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติการวิเคราะห์การถดถอยโลจิสติกแบบไบนารี

ผลการศึกษา พบว่า ผู้สูงอายุส่วนใหญ่ตัดสินใจที่จะใส่ฟันเทียม (ร้อยละ 53.7) ปัจจัยที่มีอิทธิพลต่อการตัดสินใจใส่ฟันเทียมของผู้สูงอายุ ได้แก่ ความรู้สึกจำเป็นต่อการใส่ฟันเทียม (ORAdj=21.69; 95%CI: 9.46-49.70; p<0.001) ความคาดหวังเกี่ยวกับการใช้บริการใส่ฟันเทียม (ORAdj=7.24; 95%CI: 2.44-21.44; p<0.001) อิทธิพลจากทันตบุคลากร (ORAdj=4.63; 95%CI:2.00-10.69; p<0.001) แรงจูงใจในการใส่ฟันเทียม (ORAdj=3.45; 95%CI: 1.39-8.57; p=0.008) และเขตที่อยู่อาศัย (ORAdj=2.83; 95%CI:1.04-7.66; p=0.041) โดยปัจจัยเหล่านี้มีอิทธิพลในการทำนายการตัดสินใจใส่ฟันเทียมของผู้สูงอายุเท่ากับ ร้อยละ 75.10

การศึกษานี้มีข้อเสนอแนะ คือ โรงพยาบาลควรมีนโยบายการให้บริการใส่ฟันเทียมในโรงพยาบาลส่งเสริมสุขภาพตำบลเพื่อเพิ่มการเข้าถึงการใช้บริการใส่ฟันเทียมของผู้สูงอายุ และทันตบุคลากรควรให้คำแนะนำ ให้ความรู้ ให้ข้อมูลเกี่ยวกับการใช้บริการใส่ฟันเทียม และการให้บริการที่ดีต่อผู้สูงอายุ จะช่วยเพิ่มโอกาสในการตัดสินใจใส่ฟันเทียมของผู้สูงอายุได้มากขึ้น
